Every modern toothbrush divides your teeth into four quadrants and sets a two-minute pace. In QUADRANT 120, your set of teeth lies right in front of you on the edge of the bathroom sink as a single double-sided poker-sized card. Designed specifically to be played one-handed while you brush your actual teeth with the other hand, you use your free hand to maneuver four "Plaque Dice" across the eight tooth spaces on the card.

The game is played over six rounds, tracked by a fifth die in the central Brush Zone. Each round, you must scrub by moving one Plaque Die to an orthogonally adjacent empty tooth space. Moving vertically within the left or right jaw side tips the die 90 degrees in the direction of movement, changing its top value. Sliding a die horizontally to the opposite side of the jaw keeps its value unchanged, but triggers a mandatory vibration echo that forces you to tip another die in place. After moving, you may trigger the specific tooth effect of the space you landed on: Polish (rotate a die horizontally to realign its faces for future tipping), Grind (adjust an adjacent die by +1 or -1), Floss (swap two dice within the same vertical column), or Bite Force (tip any die in the lower jaw by 90 degrees).

At the end of Round 3 or Round 4, you have the optional once-per-game choice to perform a "Rinse Flip." You flip the card from Side A (Basic Cleaning) to Side B (Fluoride Sealing), place the dice back onto their exact positions, and immediately flip one die 180 degrees to its opposite side.

After six rounds, your two-minute brushing session ends and you tally your score. Basic Cleaning rewards low die values (1–2) in the upper jaw, high values (5–6) in the lower jaw, and balanced distribution across all four quadrants. Depending on which side of the card is active, you score bonus points: Side A offers safer points for horizontal mirror symmetry (matching pairs), while Side B demands precise sums (such as the 7-Fluoride Bridge and Vertical 7-Pillars) and penalizes half-cleaned teeth showing a 3 or 4. Your goal is to achieve the highest possible score and earn the "Perfect Hollywood Smile" rating by the time you finish brushing your teeth.
