In Mightmare, players take on the role of explorers who are guided by the sorrowful voice of a girl named Mare into a deep, dark nightmare realm. To escape this world, the explorers must traverse the map, fully repair four lighthouses to restore their light, and unlock the door back to reality. However, as the nightmare deepens, one or more players may be consumed by the darkness, transforming into enemies whose new goal is to hunt down their former allies and prevent their escape.

The game is played over two distinct phases. During the first half, the game functions as a cooperative experience. On their turn, players spend action points to move across areas, draw cards, and play corresponding colored cards at lighthouse locations to advance their repair progress. While players can freely communicate and strategize, their individual actions and choices secretly accumulate "Mare Points." At the end of the fourth round, these points are calculated, and players who meet specific conditions unexpectedly turn into the enemy.

The second half of the game shifts into an asymmetrical chase. The newly revealed enemy players control the nightmare, attempting to anticipate the survivors' movements and reduce their health to zero. Meanwhile, the remaining survivors must evade these attacks, finish repairing any remaining lighthouses, and reach the exit door. If a survivor's health is depleted, they are temporarily incapacitated but can revive after a turn of rest. The game also includes a fully cooperative mode designed specifically for two players.
